Job Description

RVNAhealth is seeking an Intake Triage Registered Nurse to join our Intake Care Coordination team. In this vital role, you will serve as the first clinical point of contact for referrals across all service lines-Home Health, Hospice, and Private Duty Nursing-ensuring patients receive timely, appropriate, and high-quality care.

Schedule: Full time position - Monday through Friday, 8:30am - 4:30pm

Work Location: This is a hybrid position, with time split between:

  • Field Visits at patient homes & Facilities in Fairfield County,
  • Our offices in New Milford or Ridgefield, CT,
  • and Remote work.

What You'll Do

As an Intake Triage RN, you'll use your clinical expertise and customer service skills to:

  • Review, accept, and close referrals for care from all sources across service lines, assigning patient acuity levels and ensuring smooth onboarding.

  • Conduct clinical reviews for Home Health, Hospice, and Private Duty Nursing referrals, managing high-risk and complex cases.

  • Conduct visits with patients and families in their homes or within facilities.
  • Evaluate hospice eligibility, engage in goals-of-care conversations, and educate patients and families about the hospice benefit.

  • Coordinate care and communication between physicians, community partners, and clinical teams to develop effective, patient-centered care plans.

  • Ensure compliance with timeliness and clinical standards while maintaining detailed documentation and referral coding.

  • Participate in Intake Coordination meetings, contributing insights, identifying trends, and helping drive operational excellence.

  • Represent RVNAhealth with professionalism and compassion in every interaction.

What We're Looking For

  • Active RN license in the State of Connecticut (required).

  • Clinical and coding experience with strong understanding of home health, hospice, and community-based care.

  • Comprehensive knowledge of coverage criteria, regulations, and accrediting standards for home and hospice services.

  • Outstanding customer service and communication skills-able to connect empathetically with patients, families, and providers.

  • Strong organizational and multitasking abilities, with a calm, professional approach under pressure.

  • Tech-savvy, with comfort using agency software and electronic workflows.

  • A proactive, flexible, and solution-oriented mindset that thrives in a collaborative environment.

  • Must be able to travel throughout assigned territory of Fairfield County.
