SCAH-Sutter Care at Home - Valley
**Position Overview:**
Assists with ensuring bereavement services are provided to the families/caregivers of hospice patients in accordance with the interdisciplinary plan of care and in alignment with SCAH, mission, goals and objectives. Bereavement counseling consists of counseling given to the individual's family after the individual's death. Bereavement counseling is a required hospice service, provided for a period up to one year following the patient's death. Bereavement counseling will ensure that the bereavement services reflect the needs of the bereaved.

**FULL-TIME DAY SHIFTS: Monday - Friday / No Weekends**
**EDUCATION:**
+ _Equivalent experience will be accepted in lieu of the required degree or diploma._
+ Master's: In social work or related field
**CERTIFICATIONS AND LICENSURES:**
+ DL-Valid Drivers License
+ Automobile Insurance
+ BLS- Basic Life Support Certification
**TYPICAL EXPERIENCE:**
+ 2 years recent relevant experience
**SKILLS AND KNOWLEDGE:**
+ Understanding of psychosocial implications of illness, hospice and/or home care death and dying issues and dynamics of grief.
+ Knowledge of Medicare, Accrediting bodies, State and all other regulations applicable to Bereavement programs
+ Knowledge of community resources available to bereaved persons.
+ Excellent computer skills.
+ Must be able to discern sensitive issues and respond appropriately.
+ Must be sensitive to the human needs and situations encountered in the provision of hospice and bereavement care.
+ Must be detail-oriented and creative.
+ Must have excellent organizational skills, interpersonal skills, problem solving abilities, ability to prioritize work.
+ Must be able to work effectively with a minimum of supervision.
+ Must be able to work under time-pressure and changing priorities and be able to deal with potentially conflicting demands.
+ Demonstrate professionalism when interacting with clients and co-workers.
+ Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, and to effectively solve unique problems as they arise or identify when to consult supervisor.
+ Must be able to deal with a challenging work environment from time demands and conflicting priorities.
+ Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.
+ At orientation and annually thereafter, must be able to meet core competencies for position and area of specialty, as appropriate.

Pay Range is $40.66 to $54.60 / hour
_The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs.
