Health Ministries Clinic (HMC) is seeking a compassionate and experienced Family Medicine APRN to join our integrated care team in Newton, Kansas. This is a full-time or part-time outpatient position with limited on-call duties and a set schedule.

As part of a Federally Qualified Health Center (FQHC), you'll deliver outpatient primary care in collaboration with physicians, behavioral health providers, and nurses. You'll focus on whole-person, evidence-based care-without the pressures of excessive on-call duties or high-volume productivity models. Supported by a strong clinical team and modern in-house resources, you'll be empowered to make a real impact on the lives of individuals and families in our community.

What You'll Do

• Provide direct outpatient primary care services to patients of all ages
• Use evidence-based protocols to manage acute and chronic conditions
• Foster long-term relationships with patients and support lifestyle changes
• Collaborate with physicians, behavioral health providers, and nursing teams in an integrated care model
• Participate in quality improvement and risk management activities
• Document care using our electronic health record system

Qualifications

• Active APRN license in Kansas (or eligible)
• Minimum 5 years of clinical experience
• Commitment to providing care to patients of all ages and backgrounds
• Excellent communication and teamwork skills
