Job Description
We are seeking an Intake Nurse to join our team at an OB clinic, supporting both the clinic and a Care Coordination team for 32 hours per week. This role involves providing patient education, triage, and case management. The position also includes managing patients' medical records, referring them to social services and programs when applicable, and working closely with interpretive services to serve a diverse patient population.
Responsibilities
+ Conduct intake assessments for the OB clinic and the Chronic Disease clinic.
+ Support the Care Coordination team in delivering patient care.
+ Assist the Telephonic Triage team located on-site.
Essential Skills
+ Proficiency in medical records management.
+ Experience in scheduling and coordinating patient care.
+ Ability to operate appropriate patient care equipment.
+ Competence in accessing and entering computerized information.
+ Skills in case management and triage.
Additional Skills & Qualifications
+ Current Minnesota RN licensure.
+ Current BLS/CPR certification.
+ Prior experience in intake assessments is strongly preferred.
+ Spanish-speaking ability is highly preferred.
Work Environment
The position requires working on-site Monday through Wednesday and Friday from 8:30 AM to 5 PM, totaling 32 hours per week.

Pay and Benefits
The pay range for this position is $36.00 - $45.00/hr.
