Job Description

At My Life My Home, their mission is to passionately support each person in living a meaningful and satisfying life!

Through My Life My Home, individuals with developmental disabilities are provided support to live as independently as possible within the community. My Life My Home manages eight Community Integrated Living Arrangements (CILA) throughout St. Clair and Clinton Counties in Illinois. The individuals who reside at the CILAs are provided daily supports from a team of dedicated Direct Support Professionals, this may include personal care, cooking and assistance with meals, personal hygiene and grooming, cleaning inside their home, supporting each care plan and daily documentation.

The QIDP's primary purpose is to ensure that the individuals for whom you support are provided Person Centered Planning and Care including providing the individualized services needed to empower them to make choices of services desired, to enhance community inclusion while encourage individuals to live as they choose with integrity, dignity and quality of life. This position is responsible for providing a meaningful and satisfying life for all residents.

FT Salary position: Hours will mainly be M-F 8 am- 4:30pm/Shift times may be flexible and will include on call responsibility.

EDUCATION & SKILLS:

Must have at least one year of experience working directly with individuals with a developmental disability. In addition, you must have a Bachelor's degree in Social Work, Sociology, Special Education, Psychology, or other closely related human services field or be a licensed Register Nurse. Other degrees and professional licenses/certificates may be considered if they meet the requirements set forth by the Illinois Department of Human Services Division of Developmental Disabilities QIDP Job and Education Requirements Checklist. Must be proficient in the use of computer and programs utilized by the facility. The QIDP, must have the ability to communicate effectively in English both orally and in writing with individuals, families/guardians, other team members, physicians, and other medical professionals while maintaining HIPAA responsibilities. Must possess the desire to care and advocate for individuals at the highest level of quality. Continuing education and training is mandatory. Additionally, the ability to take initiative, make independent judgments, and promote teamwork is required to successfully fulfill this job's responsibilities.
