Job Description

Registered Nurse - Street Medicine

The Street Medicine RN will provide nursing care to individuals experiencing homelessness directly in their living environments, including parks, encampments, and under bridges. This role involves working alongside a multidisciplinary team to deliver trauma-informed care, health education, and support to a diverse and vulnerable patient population. The RN will play a key role in reducing barriers to healthcare by providing services to those who may not access traditional healthcare settings.

 

This role is on-site in Midtown Sacramento, CA (95811).

 

Essential Functions

  • Provide comprehensive nursing care, including assessment, triage, interventions, and follow-up, to unhoused individuals in non-clinical environments such as streets, shelters, and encampments.
  • Collaborate with medical providers, behavioral health professionals, case managers, social workers, and outreach workers to develop and implement patient-centered care plans.
  • Administer medications as prescribed by providers, including oral, inhaled, intramuscular, intravenous, and wound care treatments in field settings.
  • Facilitate access to care by coordinating follow-up appointments, referrals, lab testing, and specialty services for patients.
  • Provide patient education on harm reduction, chronic disease management, and medication adherence.
  • Document all patient interactions, assessments, and treatments accurately and in a timely manner in the electronic medical record (EMR).
  • Utilize nursing protocols to offer telephone or in-person triage for patients, using industry-recognized standards and tools approved by the Chief Health Officer.
  • Participate in post-hospital discharge follow-up for patients to prevent complications and readmissions, coordinating care transitions and follow-up visits.
  • Maintain compliance with safety standards and universal precautions in all patient encounters, particularly in non-clinical settings.
  • Foster a person-centered, culturally competent approach to care, building trust and rapport with a diverse population experiencing homelessness.

Additional Duties

  • Participate in street rounds with the outreach team and attend case conferences and team meetings as required.
  • Assist with coordinating care and referrals with external agencies, including shelters, harm reduction programs, and mental health services.
  • Provide active support to ensure continuity of care, including follow-up on lab results, EMR messaging, and patient communications within established timelines.
  • Report suspected cases of child, elder, dependent adult, or domestic abuse in compliance with applicable legal mandates (Penal Code, Section 11166, Section 15630, and Section 1160).
  • Perform other duties as assigned by the Nurse Manager or Medical Leadership.

 

Travel Responsibilities

  • Primarily local with occasional travel to community sites and partner organizations
  • Traveling to encampments, shelters, and other sites to meet the needs of the unhoused individuals.
  • Candidate should possess reliable transportation, valid driver's license, and car insurance.

 

Minimum Requirements

  • Associate's degree in nursing (ADN) required;
  • Current, active RN license to practice in the state of California; 1 year of clinical experience.
  • Active CPR or Basic Life Support (BLS) certification.
  • Familiarity with Electronic Medical Records (EMR), Epic experience preferred.
  • Demonstrated understanding of trauma-informed care, harm reduction principles, and the healthcare needs of marginalized populations.

Preferred Background

  • Experience working in Federally Qualified Health Centers (FQHCs) or community health settings.
  • Bachelor's degree in Nursing (BSN) preferred.
  • Previous experience in acute care settings, with proficiency in triage and management of complex medical and social conditions.
  • Experience in community health, street medicine, or working with unhoused populations.
  • Passion for serving vulnerable populations and a commitment to improving health equity.
