At our Federally Qualified Health Center (FQHC) in Palmetto, we're looking for a Chronic Care Manager to join our mission-driven care team and become a lifeline for patients navigating ongoing health challenges like diabetes, hypertension, COPD, and heart disease.

🩺 What You'll Do:

• Serve as the go-to contact for patients with chronic conditions

• Make frequent outbound calls to check in on medication adherence, appointments, and lifestyle goals

• Track and document care plans, patient progress, and clinical updates in our EMR

• Collaborate with providers, pharmacists, and care coordinators to close care gaps

• Provide compassionate guidance, health education, and motivation to help patients stay on course

• Identify and help remove barriers to care (transportation, access to meds, etc.)

• Schedule follow-ups and coordinate care transitions between visits

✅ You're a Great Fit If You:

• Are a Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) with an active Georgia license/certification

• Have at least 1 year of clinical experience, preferably in family medicine, chronic care, or an FQHC

• Love patient interaction and excel at building trust over the phone

• Are highly organized, self-directed, and comfortable managing a caseload independently

• Understand the importance of whole-person care and patient empowerment
