About the Position:
Community Care, Inc. is seeking a compassionate and dedicated CNA to provide home health care to our clients in the Westmoreland and Fayette county area. This is a rewarding opportunity to make a meaningful difference in the lives of our clients by providing personalized care and support.
Responsibilities Include:
- Assist client with bathing, dressing, grooming, toileting, bed mobility, transfers, and eating per individualized care plans
- Assist with medication reminders
- Document visits and patient progress accurately and timely
What do I need for this role with Community Care?
- Current Certified Nursing Assistant (CNA) in the state of Pennsylvania.
- Previous Home Health experience (preferred)
- CPR certification (required).
- Reliable transportation and valid driver's license.
- Compassionate, dependable, and able to work independently in a home setting.
