A DAY IN THE LIFE OF A RESOURCE SPECIALIST
In this position, the Resource Coordinator provides person‑centered support to older adults and caregivers by completing in‑person and phone assessments, offering options counseling on long‑term care services, benefits, housing, and financial resources, and assisting individuals and families with care planning and transitions following hospital, rehabilitation, or nursing facility stays. The role involves coordinating services and referrals with healthcare providers and community agencies, advocating for clients to ensure access to appropriate supports, and managing an ongoing caseload with accurate documentation of assessments, plans, and services to promote successful community living.
QUALIFICATIONS

WORK SCHEDULE FOR RESOURCE COORDINATOR
This position works Monday - Friday, 8:00 AM - 4:30 PM, with two in-office days per week and the remaining time spent working from home or in the community.
