All Heart Home Care is seeking a compassionate and skilled Physical Therapist (PRN) to join our home health team in the Nashville area. This role offers flexible, as-needed scheduling while providing personalized, one-on-one care to patients in the comfort of their homes.

Key Responsibilities:

• Perform in-home evaluations to assess patient needs and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, balance, and overall function
• Monitor patient progress and adjust treatment plans as appropriate
• Educate patients and caregivers on home exercise programs, safety, and injury prevention
• Complete accurate and timely clinical documentation in compliance with company policies and state regulations
• Coordinate care with physicians, nurses, case managers, and other members of the healthcare team
• Travel to patient homes within assigned service areas

Qualifications:

• Doctor of Physical Therapy (DPT) or equivalent from an accredited program
• Active Physical Therapist license in the state of Tennessee
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical assessment and communication skills
• Ability to work independently and manage time effectively
• Previous home health experience preferred but not required

Work Schedule:

• PRN (as needed)
• Flexible scheduling based on patient volume and availability
