Subjective interpretations are supported with behavioral observations.Uses correct grammar, spelling and approved hospital abbreviations; documents in a legible manner; signs, dates and uses professional designation each timeCorrections/late entries are made per guidelines + Patient assessmentMeets department guidelines for administering admission assessment when ordered.Recommends at least 3 activities provided by the specific program with times per week notedFollows guidelines for patients discharged and re-admitted within 30 daysIntegrates information from the assessment to assist with treatment planningIdentifies goals and timelines in concert with the patient's own goals and abilities + TreatmentProvides a balance and variety of groups that focus on the physical, social and cognitive needs of the patients. Activities are goal-oriented and individualized to enable each patient to achieve program/treatment goals.Demonstrates ability to select and/or modify activities taking into consideration age appropriateness, cultural, economic, social and educational needs of the patient. Regularly assesses effectiveness of programming.Communicates goals and purpose of each activity in a clear, knowledgeable and understandable manner.
