JOB DESCRIPTION:
To provide effective delivery of case management services, which ensure compliance with regulations, policies, procedures and standards set by the Agency and the Illinois Department of Children and Family Services (DCFS), meet the particular physical, social, emotional needs and permanency goals of children served, and in a manner that facilitates cross-team collaboration, cooperation and service integration across work unit, department and organizational boundaries.
WORK SCHEDULE:
Primarily, regular Monday through Friday business/program hours with scheduling flexibility to work variable hours as needed to effectively carry out duties and responsibilities; carries Agency pager at all times (24/7), to maintain on-call availability as needed for service provision and to ensure appropriate response to emergencies / crises of children served.
