Summary of the Role


The Outreach Case Manager plays a vital role in supporting the Outreach Ministries team. This role works under the direction of the Director of Outreach, and provides support to the Mobile Shower Trailer Outreach and overall street outreach efforts that addresses poverty, addiction, employment, housing, mental health and spirituality of those we serve.

Job Plan

  • Provide case management support to unsheltered individuals by assisting with vital document retrieval, completing housing assessments, connection to employment, and resource navigation.
  • Collaborate with community partners to connect individuals to resources for housing, community mental health, substance abuse, medical care, and mainstream resources.
  • Work closely with the Outreach Director and Facilities Department to address any maintenance issues with the outreach van and equipment.
  • Restock and order all necessary supplies and equipment needed to successfully sustain outreach programing
  • Have an understanding and working knowledge of resources directly related to housing and employment in Grand Rapids.
  • Inform those we serve of the services available to help them in their present situation. Ensure they have access to information on housing and other options to include program options.
  • Effectively engage with clients using outreach best practices from a housing first approach.
  • Serve as an active training resource for volunteers participating in Outreach shifts to ensure the highest quality experience for all involved.
  • Ensure all staff, volunteers and interns are interacting with those we serve in a grace-based and relationally driven manner.
  • Maintain accurate and timely records and ensure the physical security and maintenance of assigned equipment.
  • Maintain a positive, pleasant, professional Christian attitude. Work with other staff to forge and model transparent and supportive Christian relationships which will serve as a leadership example for recovery program men.
  • Attend all Mission meetings and training sessions, as required.
  • Work effectively with colleagues throughout the Mission's organization to achieve ministry objectives, maintaining good communication and working relationships
  • Drive between various ministry locations as needed.
  • Any other job-related duties as assigned by supervisor.
  • Maintain consistent and appropriate case management notes and documentation
  • Support the mobile shower unit program by being on site at the mobile shower locations and assisting with transporting, cleaning and maintaining the mobile shower unit
  • Work alongside volunteers at mobile shower locations to provide basic needs and connect to community resources


Qualifications

  • BSW or MSW degree preferred
  • Minimum 2 years of related experience with marginalized population
  • Proficient in Microsoft Excel, Word, Outlook
  • Possess a valid driver's license
  • Strong communication skills and ability to problem solve and adapt to change

Working Conditions

  • Understand, speak, read, and write fluent English
  • Ability to communicate verbally and to accurately hear, with hearing correction
  • Ability to see 20/20, with vision correction
