RESPONSIBILITIES **I. JOB SUMMARY** Responsible to the Director of Nursing. Assists the social work staff in meeting the psychological needs of patients and families, with particular emphasis on discharge planning. Performs duties in accordance with policies and procedures as established by the department and Molokai General Hospital. **II. TYPICAL PHYSICAL DEMANDS** Essential: sitting, finger dexterity , seeing , hearing, speaking , frequent gripping of an object. Continuous: static gripping of an object for prolonged periods. Frequent: walking, lifting usual weight of 3 pounds; carrying usual weight of 2 pounds; reaching at shoulder level; repetitive arm/hand motions. Occasional: standing, stooping/bending, kneeling, climbing stairs, walking on uneven ground, squatting, twisting body, lifting maximum of 10 pounds; pushing/pulling usual weight of 10 pounds up to 25 pounds; carrying maximum weight of 10 pounds, reaching above and below shoulder level. Operates typewriter, calculator, computer, printer, telephone, facsimile, copier. **Ill. TYPICAL WORKING CONDITIONS/ENVIRONMENT** Both inside and outside working conditions , hazardous physical conditions. (other and/or specific physical demands, machines , tools, equipment, working conditions, etc., required in area of assignment, noted on job analysis form .) **IV. MINIMUM QUALIFICATIONS:** **A. Education/Certification and Licensure :** 1. Bachelor's degree in social work related field, preferred. **B. Experience:** 1.
