Job Description

RVNAhealth is seeking a Registered Nurse (RN) Hospice Case Manager to join our growing hospice team. In this vital role, you will be responsible for providing direct patient care and comprehensive case management for individuals receiving hospice services in their homes or care facilities. You will lead the coordination and implementation of individualized care plans in alignment with physician orders and the interdisciplinary team (IDG), ensuring the highest standard of comfort and dignity for patients and their families.  This position will cover the area of Northern Fairfield County (such as New Milford, Newtown, Southbury and surrounding towns.)

Key Responsibilities:

Patient Care & Case Management

  • Conduct admissions, comprehensive assessments, and regular reassessments of hospice patients.
  • Develop, implement, and revise patient-centered care plans in coordination with the IDG and families.
  • Deliver direct skilled nursing care and manage end-of-life symptoms in homes, SNFs, ALFs, and hospitals.
  • Monitor changes in condition, communicate updates to the care team and physician, and initiate appropriate action.
  • Support families with education, dietary guidance, and compassionate end-of-life counseling.

Clinical Oversight & Compliance

  • Maintain accurate and timely documentation in compliance with agency policies and Medicare/CHAP standards.
  • Participate in interdisciplinary team meetings and case reviews.
  • Adhere to HIPAA, infection control standards, and all regulatory and safety protocols.
  • Supervise home health aides as part of the patient's care plan.

Team & Community Engagement

  • Serve as a supportive resource to patients, caregivers, and team members.
  • Participate in on-call rotation and provide coverage during evenings or weekends as needed.
  • Assist with orientation, training, and mentoring of staff as requested.
  • Represent RVNAhealth in the community and with partner organizations in a professional and compassionate manner.

Qualifications:

  • Graduate of an accredited nursing program; BSN preferred
  • Current RN license in the State of Connecticut
  • 2+ years of nursing experience; hospice or home health experience strongly preferred
  • Ability to work independently and respond effectively in crisis situations
  • Strong clinical judgment and interpersonal communication skills
  • Comfortable with electronic health records and mobile documentation
  • Valid driver's license, reliable transportation, and willingness to travel within service area
  • Current CPR certification
  • Must complete RVNAhealth's hospice training program upon hire

Th pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
