Job Summary
Responsible for assessing, treating, and monitoring residents with wounds, often including pressure injuries, surgical wounds, and diabetic foot ulcers.
Essential Functions
Wound Assessment:
- Evaluate the stage, size, and type of wounds, as well as the underlying cause and patient's overall condition. Ensure rooms are kept clean and sterile.
Treatment Planning:
- Develop and implement individualized wound care plans, selecting appropriate dressings, debridement techniques, and other interventions.
Wound Care Procedures:
- Perform wound cleaning, debridement, dressing changes, and other procedures as needed, following established protocols.
Monitoring and Documentation:
- Track wound healing progress, document interventions and patient responses, and update care plans accordingly.
Education and Support:
- Educate residents and caregivers on wound care best practices, infection prevention, and the importance of self-care.
Collaboration:
- Work closely with physicians, other nurses, therapists, and other healthcare professionals to ensure coordinated care.
Quality Improvement:
- Participate in quality improvement initiatives related to wound care practices and outcomes.
Supply Management:
- Ensure adequate supplies and equipment are available for wound care procedures.
Communication:
- Communicate effectively with residents, families, and other healthcare professionals about wound care progress and any concerns.
Compliance:
- Adhere to all relevant regulations and standards, including infection control guidelines and facility policies.
Qualifications
- Current LPN license to practice.
- Valid CPR certification.
- Must be able to safely and successfully perform job-related functions, with or without reasonable accommodation required by federal, state, or local law.
