The length of their stay is 23 days, which provides an opportunity for clinical staff to analyze a patient's behavior and cultivate a relationship that may help a patient see him- or herself in a better way. During their stay, patients will receive help with medication management and rehabilitation services if needed. Staff members will also assist in engaging patients in group activities and one-on-one exercises.

About the Households/ Long term Care Unit:
Provides long-term care for patients. Households are comprised of six units with 14 private bedrooms in each. Each HH has a central kitchen where a hostess can cook for the patients daily, den with books and a large-screen television, and a washer and dryer so that staff can wash residents’ clothing separately. HH 5-6 are for patients that may be suffering from dementia or reduced cognitive abilities; due to this staff members must badge-in and badge-out to prevent patients from exiting the units. Staff members must be able to recognize an individual’s likes, dislikes, routines and needs and deliver care based on that information.
JOB SUMMARY:
Provides safe, age-appropriate patient care, assisting with Activities of Daily Living while maintaining a safe and clean environment.
Tasks include but are not limited to:
Provides turning and positioning every two hours when indicated.
Performs routine treatment procedures including application of devices for heat, cold, etc.
Takes temperature, pulse, respiration, weight, height and blood pressure accurately and in a timely manner.
Performs re-takes without reminder.
Prepares patient for meals. Assists with meals or feeds patients.
Distributes drinking water and nourishment to patients in a timely manner.
Answers call lights in a timely manner.
Anticipates patient needs.
Make rounds of assigned patients.
Assists with handling of care of clothing and other personal property (glasses, dentures, contact lenses, hearing aids and prosthetic devices, etc.).
Collects and transports specimens according to procedure.
Maintains a safe environment for the resident: bed in low position, side rails up per policy, call light within patient's reach, etc.
Performs ROM, ambulation, wheelchair mobility, bowel and bladder training.
Applies splints properly and according to schedule.
Serves and removes food trays in a timely manner.
Obtains temperature, pulse, respiration, blood pressure, height and weight for new admission upon arrival.
Observation and Reporting:
Observes patients and reports changes to supervision.
Tasks: Identifies and notifies Supervisor/Charge Nurse of common health concerns associated with the resident.
Observes change in patient behavior or condition and notifies appropriate personnel in a timely manner.
Alerts nurse of abnormal vital signs immediately.
Alerts nurse/charge nurse of patient refusal of any procedure or task assigned in a timely manner.
Reports pain promptly.
Reports complaints and unusual incidents to appropriate personnel promptly.
Responds appropriately in emergency situations.
REQUIREMENTS:
GED equivalent or High School certificate.

About the Households/ Long term Care Unit:
Provides long-term care for patients. Households are comprised of six units with 14 private bedrooms in each. Each HH has a central kitchen where a hostess can cook for the patients daily, den with books and a large-screen television, and a washer and dryer so that staff can wash residents’ clothing separately. HH 5-6 are for patients that may be suffering from dementia or reduced cognitive abilities; due to this staff members must badge-in and badge-out to prevent patients from exiting the units. Staff members must be able to recognize an individual’s likes, dislikes, routines and needs and deliver care based on that information.
JOB SUMMARY:
Provides safe, age-appropriate patient care, assisting with Activities of Daily Living while maintaining a safe and clean environment.
Tasks include but are not limited to:
Provides turning and positioning every two hours when indicated.
Performs routine treatment procedures including application of devices for heat, cold, etc.
Takes temperature, pulse, respiration, weight, height and blood pressure accurately and in a timely manner.
Performs re-takes without reminder.
Prepares patient for meals. Assists with meals or feeds patients.
Distributes drinking water and nourishment to patients in a timely manner.
Answers call lights in a timely manner.
Anticipates patient needs.
Make rounds of assigned patients.
Assists with handling of care of clothing and other personal property (glasses, dentures, contact lenses, hearing aids and prosthetic devices, etc.).
Collects and transports specimens according to procedure.
Maintains a safe environment for the resident: bed in low position, side rails up per policy, call light within patient's reach, etc.
Performs ROM, ambulation, wheelchair mobility, bowel and bladder training.
Applies splints properly and according to schedule.
Serves and removes food trays in a timely manner.
Obtains temperature, pulse, respiration, blood pressure, height and weight for new admission upon arrival.
Observation and Reporting:
Observes patients and reports changes to supervision.
Tasks: Identifies and notifies Supervisor/Charge Nurse of common health concerns associated with the resident.
Observes change in patient behavior or condition and notifies appropriate personnel in a timely manner.
Alerts nurse of abnormal vital signs immediately.
Alerts nurse/charge nurse of patient refusal of any procedure or task assigned in a timely manner.
Reports pain promptly.

The length of their stay is 23 days, which provides an opportunity for clinical staff to analyze a patient's behavior and cultivate a relationship that may help a patient see him- or herself in a better way. During their stay, patients will receive help with medication management and rehabilitation services if needed. Staff members will also assist in engaging patients in group activities and one-on-one exercises.

What You’ll Do
As a City Year AmeriCorps member, your service will focus on coaching and mentoring elementary, middle and/or high school students. You will plan and monitor individual student progress and work with teachers on whole class instruction, breakout spaces and other in-class student assistance.
Role and Responsibilities
Experience in all these tasks is not necessary. We provide 2-4 weeks of training at the beginning of the year to support you. Plus, you will work with partner teachers and school staff every step of the way.
Support academic coaching and social emotional development for students:
Work with identified students 1:1 and in small groups
Develop literacy and/or math skills varying learning activities by student level, answering student questions and encouraging assignment completion
Create and implement a behavior management system for students
Develop social-emotional skills throughout their day, using proven strategies of learning, practicing, reflection and repetition
Collaborate with teachers and team members to collect, record, analyze and monitor individual student progress, participating in facilitated reviews of student data collected and entered into City Year’s student data system
Create an engaging, safe and healthy classroom space in collaboration with the teacher
Support Whole School initiatives to increase engagement with students and families:
Support and/or conduct extended day and virtual programming for students before, during and after school including, but not limited to, academic enrichment and intervention, homework support, recreation and student clubs, service learning, and leadership development
Create, support and/or co-lead events or programs that recognize and appreciate student growth and/or achievement in math, literacy, attendance, and behavior
Engage students, parents/guardians, and/or community members in education or social issues through member led themed events at schools or on virtual platforms

UNIT SUMMARY:
ICU Step Down- HICU is a 40-bed ICU step down unit which takes care of patients needing close monitoring and frequent interventions. Our patients are stable but critically ill and can change in condition rapidly. Nurse to Patient Ratio is 1:4with support from Patient Care Techs. Each room is semi-private, and some rooms are equipped with telemetry monitoring. Our unit consists of patients requiring acute rehabilitation, support for chronic conditions and ventilator support.
We take care of a wide variety of complex medical patients with acute and chronic disorders including post cardiac arrest, pneumonia, sepsis, acute hypoxic respiratory failure, CVA acute rehabilitation and other respiratory, cardiac renal and GI/GU conditions. Patients on our unit have specialty equipment and devices including G-tubes, PICC lines, chest tubes, High flow Nasal Cannula, tracheostomies, mechanical ventilation. blood transfusions and non-titratable drips.
Our nurses will be trained in a fast-paced environment and will be ACLS certified to be able to provide care for the patient population. We have dedicated teams of providers and respiratory therapists that are on the unit to assist in the care of our patients. While HICU is critical and fast-paced, we have stellar teamwork.
