Responsible for providing support in performance of activities within production facilities to support the manufacture of Master and Working Cell Banks Cell & Gene Therapy products, and final product Job Summary
Reporting to the Director of Community Based Health Services, the Coordinator provides program coordination related to HIV prevention/care and Substance Use Disorder (SUD) related programs. This position supports the Health Access and Service Delivery Team in providing comprehensive education, assessment, intake, referrals, linkages to treatment and follow-up for HIV prevention/care and SUD, primarily for Communities of Color to include Black and Hispanic/Latinx youth and young adults aged 13-24. Bilingual Spanish-speaking proficiency is required.
The Coordinator will function as a community health educator and health advocate for clients who are being navigated through the continuum of care through SUD and HIV prevention projects with specific focus on the SAMHSA funded Substance Abuse and HIV Prevention Grant. Through building relationships and rapport with clients, acting as a liaison between HIV and SUD providers, and identifying resources for clients to overcome barriers to care, the Coordinator helps ensure that clients are engaged in prevention and care for HIV and SUD. The Coordinator will address related topics such as HIV prevention, Pre and Post-exposure Prophylaxis (PrEP and PEP), SUD, contraception, pregnancy, abortion, reproduction, behavioral health, Sexually Transmitted Infections (STIs), prenatal health, among others. The Coordinator will comply with AccessMatters policies and commitment to maintaining a safe and healthy work environment. In pursuit of this goal, the Coordinator will follow OSHA standard 29 CFR 1910.1030 if exposed to bloodborne pathogens to get tested and report any injury to AccessMatters. The Coordinator executes activities to support the agency’s Health Access and Service Delivery programs.
The Coordinator will support the achievement of program goals, which include providing patient navigation services; documenting participation, surveys and referrals for reporting and evaluation purposes; and helping to maintain strong partnerships with the provider network. The Coordinator will support program operations such as scheduling all quarterly provider network meetings under supervision of the Director, CBHS, monthly provider meetings and scheduling committees of AccessMatters staff responsible for operationalizing SAMHSA grant workplan activities.
The Coordinator also provides sexuality education, sexual health and risk-reduction presentations to youth, young adults, adults, and professionals, and represents AccessMatters at external outreach and education events as assigned. In addition, the Coordinator assists the Director with program operations, maintains strict confidentiality, and appropriately documents client contacts, referrals, and services provided. This position will be a member of the larger AccessMatters Health Access and Service Delivery department and is expected to participate in all department and agency activities as needed. This job is full-time and non-exempt.
Essential Functions:
Reporting to the Director, Community Based Health Services, the Coordinator, Community Based Health Services will:
Serve as Lead Navigator for the SAMHSA Substance Abuse and HIV Prevention Grant
Assist the Project Director by serving as point of contact for execution of activities delivered through SAMHSA’s Substance Abuse and HIV Prevention Grant, including scheduling client services, event staffing, custody of client records and incentives, event logistics, and community engagement.
Establish relationships with and serve as point of contact for clients who are being navigated through a continuum of care through SAMHSA's Substance Abuse and HIV Prevention grant.
Function as a community health worker to provide information services, consultation, case management, one-on-one counseling, and referrals to substance use, HIV, and sexually transmitted infection (STI) screening, testing, prevention, and treatment; facilitate, schedule, providing reminders for, or attend client appointments, as necessary.
Provide sexual and reproductive health education and referrals around HIV, STIs, hepatitis, and other sexual health issues, to improve client health literacy, increase active participation in care decisions, and to maintain health care access.
Identify and address barriers to care, including transportation, insurance, language, and childcare arrangements, and coordinate and/or refer clients for additional supports (e.g., housing, employment, legal services) as necessary.
Create and implement individualized plans for connection to ongoing care for clients engaged in patient navigation services; manage follow up visits to ensure continuation of care and health promotion.
Maintain accurate and confidential case management records to track patient navigation services, client encounters, missed appointments, and other information as appropriate, including weekly logs, sign-in sheets, or other documentation of participation and referral tracking.
Adhere to all program guidelines and protocols and protect client information and records at all times in compliance with agency Confidentiality Policy, HIPAA Privacy Standards, and Pennsylvania’s Act 148.
Engage in Community Outreach and Program Promotion
Collaborate with Project Director to help maintain strong relationships and communication with potential and existing SAMHSA, Title X, RWPD and Substance use treatment providers.
Serve as outreach coordinator to promote the program in communities at risk and identified in planning, in community clinics, organizations and institutions that are stakeholders in the improvements of reproductive and sexual health, through outreach, presentations, and lectures.
Coordinate and participate in supplementary activities including: research, attendance at external trainings, community education and health fairs, outreach, site visits and others as assigned.
Distribute educational and informative materials about HIV, substance use, and support services to community partners and other organizations.
Assist in the production, packaging, and distribution of educational materials and promotional items directed toward youth, including fact sheets, brochures, condom packets, promotional pieces, and other publications.
Maintain communication with relevant health care and support services entities that may provide diagnostic, treatment and additional social support for clients.
Attend meetings with behavioral health providers, designated family planning providers, and other relevant stakeholders to maintain progress on the project.
Represent AccesssMatters on coalitions and committees as relevant to agency agendas and mission.
Stay informed of current information, trends, and best practices in the fields of youth development, education, adolescent, reproductive, and sexual health; stay current on relevant topical information and changes in services at key providers.
Coordinate Program Operations and Goal Attainment
Maintain close communication with supervisor and meet regularly with program team to report and review progress.
Schedule and in partnership with Project Director, create agendas for all Quarterly SAMHSA Partner meetings, monthly partner check-in meetings and meetings of internal staff regarding completion of SAMHSA project workplan goals (such as focus group and media campaign planning).
Submit data, reports, and case files in a timely manner to track progress, address concerns, and ensure objectives are being met.Collect and process program data and paperwork, maintain and regularly update program database in an accurate and timely way, and generate reports as required by management.
Complete routine administrative duties including filing, shredding, collating, stocking, and other operational needs.
Maintain high ethical standards and conduct regarding confidentiality, integrity, dual-relationships, and professional behavior overall.
Participate in special events, trainings, programs, projects, meetings, and other duties as assigned.

Represent AccessMatters on coalitions and committees as relevant to agency agendas and mission.

Experience working with individuals managing behavioral health disorders including substance abuse, and other marginalized populations who face barriers to accessing treatment.
Bilingual Spanish-speaking proficiency required.

EDUCATION AND EXPERIENCE
BA/BS in Health Education, Public Health, or related field preferred. Relevant industry specific experience may be considered in lieu of degree where possible.
Previous experience working in counseling, community-level, or client-level health education, especially in the fields of HIV prevention and/or care, SUD, sexual and reproductive health; HIV/STI testing, prevention, and counseling services; and/or youth development.
Previous experience working with youth and young adults (ages 13-24) in counseling, community-level or client-level health navigation or education, especially in the fields of HIV/STI testing, prevention, and counseling services, SUD, sexual and reproductive health, family planning and maternal health.

Essential Duties and Responsibilities
Resident Service Coordination
Work with Central Intake and Property Management to coordinate assessment, intake, and orientation of new residents. Coordinate resident discharges to include appropriate services and supports.Work with staff and residents to assess needs, goals, barriers, stability and progress in the areas of recovery, health, education, employment, parenting and/or community integration. Oversee resident emergencies and urgent situations, including 302 petitions.Oversee medication room management, as applicable.
Leadership
Create an environment with the necessary structure, service coordination support, interaction, freedom, and safety to facilitate resident growth, individuality and connection to Project HOME and the external community. Maintain positive relationships with service providers, the public, and volunteer groups.Provide regular, consistent individual and group supervision, team meetings, and general oversight of residential site staff to ensure residents receive services appropriate to their needs. Oversee staff training plans, including required trainings and skill-building trainings. Create a person-centered environment with a focus on DEI and a culture of recovery and wellness.
Administration
Work with Directors and Data and Quality Assurance Department to ensure all documentation meets contractual obligations, Strategic Plan goals and KPIs. Monitor and assure compliance with electronic data system standards. Recruit and onboard staff. Manage the on-call schedule and site schedule.
Financial Management
Manage the day-to-day financial operations of the site, including annual budget and program spending, petty cash, SNAP report, purchase of goods and services, and credit card.
Coordination with Property Management
Work as a blended management team with the Property Manager:
follow-up with any leasehold obligation issues (rent arrears, inspection violations, housing instability and incidents), and plans to address these issues.address residence cleanliness and safety, to assure the health and safety of residents and staff and pass all inspections and comply with safety and licensing regulations. ensure that housing re-certifications, PHA income waivers and other documentation issues are completed in a timely manner.
